Athletic Forms

All students playing sports at Fairfield Prep must register online. Part of the registration process will require that the player and parent read and agree to several guidelines and consent and behavior policies which can be read on the website.  You may download the forms for your personal use and information.  Hard copies of forms are no longer accepted as registration. Please go to the Registration Page for the sport of your choice, found in the left navigation panel.
